Ghastly Gibus je kosmetický předmět pro všechny třídy. Vypadá jako poškozený černý cylindr se skoro odděleným vrchem.

Podobně jako u Gentle Manne's Service Medal, prvním způsobem jak získat tento klobouk bylo navštívit jako jeden z 10,319 lidí tajnou webovou stránku. Druhá metoda byla dokončit Ghastly Gibus Grab achievement. Na rozdíl od Mildly Disturbing Halloween Mask mohou hráči tento klobouk nosit po celý rok.

Každý Ghastly Gibus získaný před Scream Fortress updatem byl změněn na Ghastlier Gibus.
